Skin Friction
Clinical Condition:
Two surfaces rubbing together and wearing away the outer layers of the
skin. This type of injury is often seen on the elbows or heels due to
rubbing against bed sheeting and/or from re-positioning in bed. It also
can occur on the buttocks or sacrum from sliding down in bed if the head
of the bed is elevated.

Ulcer Stage I
Clinical Condition:
A Stage I pressure ulcer is an observable pressure-related alteration of
intact skin whose indicators as compared to adjacent tissue or opposite
area of the body may include changes in one or more of the following:
skin temperature (warmth or coolness), tissue consistency (firm or
boggy), and/or sensation (pain or itching). The ulcer appears as a
defined area of persistent redness in lightly pigmented skin, whereas in
darker skin tones, the ulcer may appear with persistent red, blue or

Ulcer Stage III
Clinical Condition:
Full thickness skin loss involving damage to or necrosis of subcutaneous
tissue that may extend down to, but not through, underlying fascia. The
ulcer presents clinically as a deep crater with or without undermining
of adjacent tissue. Also applies to open surgical and traumatic wounds.

Ulcer Stage IV
Clinical Condition:
Full thickness skin loss with extensive destruction, tissue necrosis, or
damage to muscle, bone or supporting structures, i.e. tendon, joint
capsule. Undermining and sinus tracts (tunnels) may be associated with
Stage IV ulcers. Also applies to open surgical wounds or traumatic
wounds.
